4/3/2023 EnchantaVaughn Approved Behavior: Upon QMHP arrival, client was waiting by the door to greet QMHP. Client appeared to be in a pleasant mood, as evidenced by his calm and subtle facial expressions and discussions. Dad appeared to be in a good mood as well, as observed by his demeanor and verbalizations. Incidents of recent anger outbursts by Client were processed and alternative, adaptive ways to express that anger were reviewed. 1. Response:. During the session the Jahsim participated in a brainstorming activity conducted by the QMHP-C to help increase the Jahsim’s ability to manage stressful situations as a coping skill. Jahsim was encouraged the Jahsim to give himself praise when he has made an accomplishment. 2. Response: Jahsim and QMHP-C discussed topics relating to his frustrations, according to the Jahsim he becomes upset with immediate and extended family disputes the directly affect him at home. Interntion: QMHP monitored client individualized behavior modifications in collaboration with his family. QMHP modeled proper positive behavioral expectations for Client to administer. QMHP provided client with encouragement to demonstrate the positive behaviors and expectations they had been talking about. QMHP assisted client in exploring other coping strategies to utilize in the home. QMHP isolated client from distractions to decrease disruptive behaviors and to reduce episodes. QMHP assisted client in developing more social skills. QMHP inquired about possible challenges client can experience with applying this skill. QMHP provided positive feedback. QMHP reflected on client goals and objectives. Intervention: QMHP motivated client to respond to exercise and express his feelings. QMHP offered active listening skills. QMHP encouraged client to communicate to his mom when he needs help. QMHP praised client for staying on task. QMHP complimented client for utilizing acquired skills, paying attention, and focusing on the importance of the assignment. QMHP educated client about what he can do to make his daily activities more fun and interesting, such as reading his favorite book, drawing in a journal, stretching and closing his eyes and practice the mindful behaviors learned in previous sessions. QMHP assisted client in normalizing the changes that are occurring at home and community. Response: Client is making progress with expressing himself but has a few communication skills to work on as well. Client responds well to praise and support. Client is acceptive of redirection. Client has improved with staying calm and utilizing relaxation skills in the home. Client responds well to feedback. Client responds well to the positive reinforcement provided by QMHP and is seen increasing self-esteem and confidence. Client said he was going to practice coping strategies and relaxation techniques in all aspects of his daily living routine in hopes that he will be a better individual. Response: Client understands that sessions are a privilege and he have to show good work ethic and follow along with the interventions. Client displays coping skills when faced with conflict due to deep meditation exercises. Client anger is moderate to intense, 2-3x per week. Client is working on walking away and telling an adult before responding to anger provoking situations. Client understands that problem solving starts with thinking before acting. Client displayed an understanding of ADHD and its symptoms and causes. Client was observed and reported eliminating personal conflicts at home.
